The New Therapeutics: 10 Commandments

Hi! I am a Spanish surgeon and medical journalist. I contribute monthly to EL PERIODICO, a general newspaper that most probably would be interested in publishing a Spanish version of the 10 Commandements (excellent text, by the way) for the laymen. May I proceed? Should I ask a copyright permission? To whom?

Hi Carolyn,
Thank you for a fine and thought-provoking post and for helping people understand the importance of discussing treatment options with their doctors. I have a wonderful GP who respects my viewpoint on tests, some of which I have decided to forgo after careful consideration and research.

Quality of life is vital to me, and I think it’s also vital to be a participant in my own health care. The fact that drugs and treatments can never be a case of ‘one size fits all’ may not be a message drug companies want to hear, but it is very important for everyone to take some responsibility for their own situation.

Carolyn, I am honored that you appreciated my comments. I truly admire and enjoy the posts in your blog.

Let me present an idea on Medical Ethics. Most of the categories that are discussed under the title of Medical Ethics relate to issues about patients, meaning individuals who are sick. However, most of our modern ethical violations, I contend, are actions and behaviors that affect individuals who are not sick. It is in this group where the largest proportion of overdiagnosis and overtreatment is encountered.

Let me illustrate this point, Ethics Committees focus on making sure that patient’s rights are not violated, an important task, but, I ask you: when have you seen a committee concerned about the issue of overdiagnosis of prostate cancer on individuals screened with PSA? Or address the statistic illiteracy of physicians? These are issues that Gerd Gigerenzera* has argued needs to be addressed by Ethics Committees, but so far no one is paying attention. * author of Calculated Risks: How To Know When Numbers Deceive You

The surrogate issue brought out in this post is a good example, but, again, what Ethics Committee is paying attention?
